The short version: Tirzepatide is one peptide that switches on two hormone receptors, GIP and GLP-1. It is the active ingredient in Eli Lilly's Mounjaro and Zepbound, and the compound that made "dual agonist" a phrase people actually search for.
Tirzepatide is a chain of 39 amino acids. Its backbone is modelled on GIP, a hormone your gut releases after a meal, and it has been tweaked so it also activates the GLP-1 receptor. A fatty-acid tail lets it ride on albumin, a protein in the blood, which is why it lasts about five days instead of a few minutes.
GIP and GLP-1 are both incretins: gut hormones that tell the pancreas to release insulin when there is food around. The first wave of compounds, semaglutide included, only target GLP-1. Tirzepatide adds GIP on top.
Exactly why adding GIP helps is still being worked out. Researchers are looking at its effects on fat tissue and on how well the GLP-1 side is tolerated. What is not in doubt is that the combination produced bigger numbers in trials than GLP-1 alone.

SURMOUNT-1 (New England Journal of Medicine, 2022) followed 2,539 adults with obesity or overweight, without diabetes, for 72 weeks. Average body weight fell by between 15% and 21% across the three doses tested, against 3.1% on placebo.
On the strength of trials like this, the US FDA approved tirzepatide as Mounjaro for type 2 diabetes on 13 May 2022, and as Zepbound for chronic weight management on 8 November 2023.
Those results come from pharmaceutical-grade drug, given under medical supervision, to carefully chosen people. Research-grade material is not that product, and none of the trial numbers tell you what it will do outside a trial.
The effects reported most often in the trials were gut-related: nausea, diarrhoea and constipation, usually early on.
