Cagrilintide is a long-acting amylin analogue that acts on amylin receptors to reduce appetite and slow gastric emptying via a different pathway than GLP-1 agonists.

The short version: Cagrilintide is Novo Nordisk's long-acting copy of amylin, a fullness hormone made in the pancreas. It is best known as the partner in CagriSema, but it has its own trial record and now its own phase 3 programme.
Amylin is a hormone made in the pancreas that tells the brain you have had enough to eat. Cagrilintide is a long-acting version, given once a week in trials. Researchers describe it as a non-selective amylin receptor agonist, meaning it does not single out one receptor in the amylin family. That detail matters, because newer rivals such as eloralintide were designed to be more choosy.
The first big test was a phase 2 trial in The Lancet (2021): 706 adults on cagrilintide across 57 sites in ten countries, South Africa among them, compared with placebo and with daily liraglutide, an older GLP-1 drug. After 26 weeks, weight was down 10.8% at the highest dose tested, against 3.0% on placebo and 9.0% on liraglutide. Nausea, constipation, diarrhoea and injection-site reactions were the common complaints.
Cagrilintide also ran as its own arm inside the REDEFINE 1 phase 3 trial of CagriSema. Over 68 weeks that arm averaged 11.8% weight loss against 2.3% on placebo, first shown at the EASD meeting in 2025. Off the back of that, Novo started the RENEW phase 3 programme for cagrilintide alone in November 2025.
On its own, cagrilintide sits at roughly half the weight loss of the combination, and RENEW has not reported yet. It is not approved anywhere as a single drug. All of the data come from Novo's pharmaceutical material in supervised trials, which tells you nothing about a research vial.
