What is Eloralintide?

An amylin-receptor agonist. Amylin is the hormone co-secreted with insulin that signals satiety, so it sits in a different pathway to GLP-1 compounds like Retatrutide. Current research pairs the two classes rather than treating them as alternatives.

How does it compare to Cagrilintide?

Both are amylin-class. Eloralintide is the newer and more selective of the two, which is exactly why it is being studied — a cleaner receptor profile makes results easier to attribute.

Eloralintide, explained

The short version: Eloralintide is Eli Lilly's amylin receptor agonist, built to be more selective than cagrilintide. A 48-week phase 2 trial reported up to 20% weight loss, and phase 3 started in late 2025.

What it is

Eloralintide (Lilly code LY3841136) is an amylin-type peptide, the same family as cagrilintide. The difference is selectivity. In Lilly's cell tests it preferred the human amylin 1 receptor, about 12 times more than the calcitonin receptor, while cagrilintide hits both. In rats it caused less taste avoidance, a lab marker linked to nausea, than cagrilintide, and it cut food intake and body weight, mostly through fat loss.

What has been studied

A phase 1 trial in 48 healthy people tested single doses. Side effects were mostly mild, and the drug's profile fitted once-weekly use.

The phase 2 trial (The Lancet, 2025) enrolled 263 adults with obesity or overweight and no type 2 diabetes at 46 US centres. After 48 weeks, average weight loss ranged from 9% at the lowest dose to 20% at the highest doses tested, against 0.4% on placebo. Nausea and fatigue were the most common side effects, and fatigue was notable, reaching over 40% in some higher-dose groups.

Lilly then launched the ENLIGHTEN phase 3 programme. ENLIGHTEN-2 (type 2 diabetes) started in December 2025 and ENLIGHTEN-1 (no diabetes) in February 2026, with further trials in sleep apnoea and knee pain.

Where the evidence stops

One phase 2 trial is the whole efficacy record so far, run by the company that makes it, in a mostly female and mostly white US group. No phase 3 results exist yet and it is not approved anywhere. The trial results belong to Lilly's material and do not describe a research vial.

Handling and storage
