What is Tesamorelin?

Tesamorelin is a synthetic GHRH analogue that stimulates pituitary GH release. Uniquely, it has been studied in clinical settings for visceral-fat research in specific populations.

Tesamorelin, explained

The short version: Tesamorelin is a lab-made copy of the full 44-amino-acid hormone that tells the pituitary to release growth hormone, with one small chemical tweak. It is one of the few compounds in this group that became a real medicine: the US FDA approved it as Egrifta in November 2010.

What it is

The hypothalamus makes growth hormone-releasing hormone (GHRH), a 44-amino-acid peptide that signals the pituitary to release growth hormone. Tesamorelin is that full natural sequence with a short fatty chain, a hexenoyl group, bolted onto the first amino acid. It is still short-lived: the FDA label lists an average half-life of 26 minutes in healthy volunteers and 38 minutes in people with HIV.

What the trials found

People on HIV treatment often build up deep belly fat around the organs, called visceral fat. Tesamorelin was tested against placebo in two phase 3 trials in exactly this group, and a pooled analysis of 806 patients appeared in the Journal of Clinical Endocrinology & Metabolism in 2010.

After 26 weeks, visceral fat measured by CT scan fell by about 15% relative to placebo, while the fat just under the skin hardly moved. Triglycerides dropped and IGF-1 rose. Patients who carried on for a further 26 weeks kept the reduction. On the back of this, the FDA approved it as Egrifta for reducing excess abdominal fat in HIV-infected patients with lipodystrophy.

Where the evidence stops

The label is blunt about the limits. Long-term heart benefit and safety were never studied, and it is not a weight-loss drug: the label calls its effect on body weight neutral. It also flags raised IGF-1, fluid retention and glucose intolerance, and rules it out in active cancer. Every one of those trial results comes from one group of people, using the licensed product. WADA lists tesamorelin as a prohibited substance at all times.

Handling and storage
