The short version: Semaglutide is the GLP-1 drug behind Novo Nordisk's Ozempic and Wegovy. It is the most heavily tested compound in this category, with a heart outcomes trial of more than 17,000 people behind it.
Semaglutide is an engineered version of GLP-1, a hormone released by the gut that acts on appetite. It acts on one receptor only, the GLP-1 receptor. Tirzepatide adds a second (GIP) and retatrutide a third (glucagon), so semaglutide is the single-receptor benchmark the newer compounds get measured against.
STEP 1 (New England Journal of Medicine, 2021) gave once-weekly semaglutide or placebo to 1,961 adults with obesity or overweight and no diabetes. After 68 weeks, average body weight was down 14.9% on semaglutide against 2.4% on placebo. Nausea and diarrhoea were the most common side effects, mostly mild and fading over time.
SELECT (NEJM, 2023) went after a harder question: does it change heart outcomes? It enrolled 17,604 people with existing heart disease, overweight or obesity, and no diabetes. Heart attack, stroke or cardiovascular death happened to 6.5% on semaglutide against 8.0% on placebo, roughly a 20% relative drop. The catch: 16.6% stopped the drug because of side effects, double the placebo rate.

All of that comes from Novo Nordisk's own pharmaceutical product. Wegovy's prescribing information carries a boxed warning about the risk of thyroid C-cell tumours. The FDA also points out that salt forms such as semaglutide sodium and semaglutide acetate are different active ingredients from the approved drug, and it had logged 990 adverse event reports linked to compounded semaglutide by May 2026, some involving hospital stays after dosing mistakes. Research-grade material is not Wegovy, and the trial numbers do not transfer to it.
