How does it compare to Tirzepatide?

Tirzepatide targets GLP-1 and GIP only. Retatrutide adds glucagon receptor activation, which may drive greater energy expenditure — it stood out significantly in Phase 2 trials.

Research guide

Retatrutide, explained

The short version: Retatrutide is Eli Lilly's "triple agonist": one peptide that activates three hormone receptors (GLP-1, GIP and glucagon) where tirzepatide hits two and semaglutide hits one. It has now passed several phase 3 trials, but it is not an approved medicine yet.

What it is

Retatrutide (Lilly's code name was LY3437943) is a single peptide built to switch on the GLP-1 and GIP receptors, the same pair tirzepatide targets, plus a third: the glucagon receptor. Glucagon is usually thought of as the hormone that raises blood sugar, but it also pushes the body to burn energy, which is why researchers wanted it in the mix.

Why three receptors

The idea is that GLP-1 and GIP turn appetite down while the glucagon arm turns energy use up. Whether the third receptor adds as much as it seems to in trials, and at what cost, is exactly what the large trials are there to answer.

What the trials found

The phase 2 trial (New England Journal of Medicine, 2023) reported average weight loss of 24.2% after 48 weeks at the highest dose tested, against 2.1% on placebo.

Phase 3 followed. TRIUMPH-4 (December 2025), in people with obesity and knee osteoarthritis, reported 28.7% average weight loss at 68 weeks at the highest dose, along with a large drop in knee pain scores. TRIUMPH-2 and TRIUMPH-3 (July 2026) reported up to 20.8% and 22.6% at 80 weeks. Lilly plans to file for approval in early 2027.

Where the evidence stops

Every one of those numbers comes from Lilly's own pharmaceutical-grade drug, in supervised trials, in chosen groups of people. Until a regulator has reviewed the full data, nobody outside those trials knows the complete safety picture. Research-grade material is not the trial drug, and the trial results say nothing about what it will do.

Handling and storage
