Ipamorelin is a pentapeptide GH secretagogue that may selectively stimulate GH release without affecting cortisol, prolactin or ACTH — making it the cleanest secretagogue for research purposes.

The short version: Ipamorelin is a five-amino-acid peptide that prompts the pituitary to release growth hormone. Its selling point in the research is what it does not do: it does not drag cortisol and other hormones up with it, which older peptides in its class did.
Ipamorelin is a growth hormone secretagogue: something that prompts the body to release its own growth hormone rather than being growth hormone itself. It works on the ghrelin receptor, the same switch the hunger hormone ghrelin uses.
When Novo Nordisk researchers first described it in 1998, they called it the first selective growth hormone secretagogue. In their pig studies it released growth hormone about as strongly as the older peptide GHRP-6, but without raising ACTH or cortisol, even at doses far above the amount needed for the growth hormone effect.

Ipamorelin reached a phase 2 trial for a different purpose: getting the gut moving again after bowel surgery. In 114 patients, the ipamorelin group recovered slightly faster on average, but the difference was not statistically significant, and development did not go further. It was well tolerated.
The hormone-selectivity data comes mostly from animals, and the one sizeable human trial missed its main goal. There is no approved medicine containing ipamorelin.
