What is PT-141?

PT-141 (Bremelanotide) is a melanocortin receptor agonist that acts centrally via MC4R to influence sexual arousal. Unlike PDE5 inhibitors, it works through the CNS rather than vascular pathways, making it a unique research compound.

Research guide

PT-141 (Bremelanotide), explained

The short version: PT-141 is bremelanotide, a ring-shaped peptide that switches on melanocortin receptors, including ones in the brain. It is approved in the US as Vyleesi (2019) for low sexual desire in premenopausal women, and that approval is deliberately narrow.

What it is

Bremelanotide is a synthetic cyclic peptide of seven amino acids. According to its FDA label it activates several melanocortin receptors, most strongly MC1R and then MC4R. MC4R sits on nerve cells across the brain; MC1R sits on pigment cells, which is why skin darkening is a known side effect. The label states plainly that how it affects sexual desire is unknown.

It came out of melanocortin research sparked by the finding that MT-2 caused erections. Early clinical work used a nasal form and included phase 2 trials in men with erectile dysfunction.

What the trials found

The approval rests on RECONNECT, two identical phase 3 trials published in Obstetrics & Gynecology in 2019. They randomised 1,267 premenopausal women with hypoactive sexual desire disorder (HSDD) to bremelanotide or placebo for 24 weeks. Both trials showed statistically significant gains in desire scores and drops in distress about low desire.

Side effects were common. The label reports nausea in 40% of patients, a short rise in blood pressure with a dip in heart rate after each dose, and darkened patches of skin in about 1%. Far more women quit on the drug than on placebo: 40% versus 13% in one trial.

Where the evidence stops

The label limits it to premenopausal women with acquired, generalised HSDD. It is not indicated for postmenopausal women or men, and not to enhance sexual performance. The trial gains were modest, and the published work in men we found stopped at phase 2. None of it covers research-grade material.
